Submission of this form does not guarantee an approval of booking. Initial approval by HACA's Program Manager and Events Coordinator, Angela Clemmons, is required. Completed forms can be submitted online, via email to Angela, or by bringing to the Arthur Edington Center, 133 Livingston St, Room LL06.

Payment Deadlines
Deposit + $25 fee due at booking to confirm your date
Full payment due 3 business days before the event
IRS W-9 form required for deposit refund — no exceptions
